After your visit to SearsHometown be sure to complete the customer feedback survey for a chance to win a $4,000 gift card! The survey will take about 3 to 5 minutes to complete and will ask you questions about you visit and shopping experience at the Sears Hometown store. To begin you must have the SALESCHECK # and the ASSOC# on hand. Both these numbers can be located on your receipt. The sales check number will be 12 numbers long and will be located towards the top center of the receipt. The Associate number will be four digits long and located under the Sales Check number. You will also be required to enter the date of purchase. At the end of the survey you will be given the chance to enter a quarterly drawing for the gift card. Please be aware that not all computers will not be able to access the survey.
